Completing the Form
Workers should complete Birth Certificate while the prospect is completing the
Decision Card. The Worker gives the Birth Certificate to the prospect and points
out they should put it in their Bible so they will always remember that "today
was the day".
A. is for the first name of the prospect
B. is for today's date
C. should be completed by the prospect after
they have been Baptized
D. should be completed by the prospect after
they have joined a church that preaches and teaches what they heard in the soul
winning booth
E. is for the first name of the soul winner,
